Any hazard may be reported anonymously and without fear of reprisal.
We value your input into safety and health decisions.
If you elect to include your e-mail address on this form, you will be contacted with updates regarding the assessment and correction of the hazard.
Only the location and the description of the hazard are required.

- Complete a Hazard Notice
- Verbally identify the hazard to your supervisor, the IIPP administrator, or any member of management (verbal reports must be recorded to accompany the hazard assessment and correction documents).
